The Adolescents and Youth Behavioral Clubhouse (AYB) delivers an integrated clinical and faith-based model of respite care and behavioral health services to retrain young minds, transform behaviors, and unlock human potential. We equip adolescents and youth with essential cognitive, emotional, and moral frameworks for optimal outcomes while addressing systemic challenges to build resilient families and purpose-driven leaders.

IPC / Plan of Care (POC) Signing Requirements

• Guardian and youth (age-appropriate) must sign the IPC/POC within7–14 days of authorization

• Signing acknowledges acceptance of the therapeutic respite model, 3-month outcomes, MTS/Health Home integration, and out-of-pocket responsibilities

• Digital signatures accepted via a secureHIPAA-compliant platform

• Failure to sign within the timeline may delay services or result in waitlist placement

• All fees due at the start of each 3-month authorization period
